As the parent/guardian, I certify that my child has no known medical or other conditions that could interfere with their participation in Elite United Sports activities. I acknowledge that participating in sports like soccer, volleyball, basketball, tennis, baseball, and softball, whether indoors or outdoors, involves certain risks of physical injury.
Therefore, I, individually and on behalf of my child, release and hold harmless Elite United Sports, its parent company, subsidiaries, affiliates, officers, directors, agents, employees, and successors from any claims, actions, or damages arising out of my child’s participation in any Elite United Sports program or activity. I agree not to bring any legal action against Elite United Sports or any of its affiliates based on my child’s participation.
I understand that Elite United Sports has the right to terminate my child’s enrollment if it is determined that continued attendance is not in the best interest of the child or the program. I agree to supervise my child before and after activities and understand that for children aged 5 and under, I must remain in the immediate vicinity of the class. In the case of an allergic reaction, I am responsible for my child’s care. Elite United Sports coaches may administer medication or EpiPens only with a completed medical waiver, which can be obtained from customer service.
I understand that Elite United Sports coaches and staff are not permitted to assist with bathroom duties, and that responsibility lies with me or my designated caregiver.
Elite United Sports and its agents reserve the right to take and use photographs, videos, or recordings of participating children and their parents or caregivers during classes or activities. I consent to the use of such media for advertising, promotional, or related purposes without compensation and waive all rights to such media.
Elite United Sports guarantees a full prorated refund (minus a $20 administrative fee) for the remaining classes before the third class of any session. No refunds will be issued after the third class. If you wish to withdraw your child from the program, you must notify the Elite United Sports office at least 24 hours before the third class, and you will be responsible for all charges until that time. No refunds or credits are provided for missed classes.
Two free make-up classes are allowed, subject to availability and with prior consent from Elite United Sports management. Additional make-up classes are available for a fee of $20 each. All make-ups must be scheduled and canceled at least 24 hours before the scheduled time, and they must be used before the end of the current semester. Make-up classes cannot be scheduled for the first or last week of a session.

Your child’s spot in the class will be confirmed only after payment and the application are received. A $25 service fee will be charged for bounced checks or insufficient funds.
Class details and receipts will be sent via email if an address is provided.

RAIN POLICY:
We make the decision to cancel class due to rain on the MORNING of the DAY of your class If we need to cancel your class due to rain, we NOTIFY YOU VIA EMAIL that we have on file. Please check your security and SPAM settings to allow emails from admin@eliteunitedsports.com. Check your SPAM inbox in case our email went there. We also post canceled class announcements on the HOMEPAGE of our website www.eliteunitedsports.com in the bottom of the page. If you do not have access to the internet and the weather is questionable, please call our office + 1929 675 8217 to determine class status. All rained out classes will be automatically added on to the end of the season, on your usual day and time. If it is raining or the radar shows a storm coming within 2–3 hours of your class, we will cancel your class for the day.
